Referral to The Livingwell Service

Many things affect your well-being, and a lot of people feel isolated or lonely. Or they might be stressed out by work, money, and housing issues.

The Social Prescriber Link Workers are there to listen to you and put you in touch with the people and activities that might help you feel better. Social Prescribing is there to help in the prevention of escalation of a situation that could be a further detriment to both physical and mental health.

This may involve:

  • Introducing you to a community group, new activity, or a local club.
  • Help you find legal advice or debt counselling.
  • Information, guidance, and inside knowledge on your situation or what local resources there are.
  • Help to create something new such as a gardening club, a fishing group a ‘mens shed’ or knit and knatter group.
  • Signpost people with wider social issues such as housing, relationship problems and addictions.

Health and Wellbeing Coaching will help you learn new skills and build your confidence to enable you to reach your health and wellbeing goals. We work together over several sessions to explore your values in relation to your health and wellbeing and explore the reasons you want to change your health behaviours.

  • People with long-term conditions who need support to self-manage
  • Anyone with mild mental health issues who may be anxious or depressed
  • People who use the NHS the most, because their needs are complex
  • Those who struggle to engage appropriately with services and go through ‘revolving doors’
